Octo E-N counts eosinophils and neutrophils in every high-power field of the biopsy, so the diagnostic call rests on the highest field on the slide rather than the busiest one someone happened to pick.
Granulocytes are counted by hand, field by field.
The pathologist scans the biopsy, picks the fields that look busiest and counts eosinophils one by one. Counts near a diagnostic threshold depend on which fields were chosen and who counted them.
Thresholds decide the diagnosis. Eosinophilic oesophagitis and gastritis are diagnosed on counts per high-power field; neutrophils define activity in gastritis and inflammatory bowel disease. A full count, with the peak field shown, makes the threshold call reproducible.
